weight loss questionnaire

I have had a few GP’s through out my life and I was wondering if ALL my information is passed on to the ext?

I want to do a weight loss programme but your GP has to fill out a health questionnaire. If you have or have had epilepsy you cant go on to it.

Is it possible my new GP would not know about my epilepsy as i have not had a fit in 12 years or been on medication.

Or is there a way I can get around this. The programme is called lighter life.

I was with the same GP for over 30 years and moved recently to the other end of the country. When my new GP received my notes he called me to the surgery to discuss my past and recent medical history.
Afraid they are with you for life.

weight loss surgery ( lap band) called for an appointment and recieved a questionnaire first…
